Peru holds two different journeys and it matters which one you are booking. The first is the Andes, the Sacred Valley below Cusco, thin bright air near three thousand metres, Quechua villages, silence that has weight to it. The second is ayahuasca, and that one starts with your own doctor.
The place
At this altitude the first flight of stairs tells you where you are. You stop halfway, hands on knees, and everyone does it. The light is unlike anywhere lower down, hard and clean, and shadows have edges. Afternoons turn cold the moment the sun drops behind the ridge, and you feel it through wool. At night the sky above the valley is so crowded with stars it looks wrong.
Timing
The Andean dry season from May to September gives cold clear nights, bright days and the most reliable trails, and almost everything is scheduled then. The wet season from December to March brings daily afternoon rain, mud and landslide closures. The Amazon lowlands stay hot and humid all year. Give yourself two days in Cusco before anything demanding.
Layers, because the valley swings from hot sun to near freezing fast, plus a real hat and lip balm for the altitude. Broken-in shoes, a refillable bottle, and regular medication with its prescription. If ceremony is part of your plan, bring a written list of everything you take, including supplements.
The region

Most Western passports enter without a visa and receive a stay of up to ninety days, sometimes less at the officer's discretion, with onward travel often checked. Extensions are limited. Confirm the current allowance for your nationality with the Peruvian consulate before booking, since the granted period is decided at the border.
These are ceremonies with a powerful psychoactive brew, legal in Peru within traditional ceremonial use, illegal to carry home almost everywhere else. The vine acts as a monoamine oxidase inhibitor, so combining it with antidepressants, particularly SSRIs and MAOIs, carries a real risk of serotonin syndrome, and a personal or family history of psychosis or bipolar disorder is a genuine contraindication. It also pushes up blood pressure and heart rate, so uncontrolled hypertension and heart disease belong on the same list, and the brew must never be combined with anything else, alcohol and stimulants included. The sector is largely unregulated. People have died and people have been assaulted by facilitators. Who is running the ceremony, what medical screening they insist on and who is awake in the room are safety questions about your life, not preferences. If you would not answer them for a surgeon, do not skip them here. By what they refuse. A serious place screens your medications and psychiatric history in writing before accepting you, turns people away, states who is trained in first aid, keeps sober staff awake through the night and never mixes strangers with no intake. Vagueness about any of that is the answer. Talk to your own doctor first, especially about antidepressants, which must never be stopped abruptly on a stranger's advice.
Completely, and many people do. The Sacred Valley runs silent weeks, breath and movement practice, walking retreats and work with Andean weaving and offering traditions that involve nothing psychoactive at all. The altitude alone will slow you down more than you expect. Choosing that version is not a smaller trip.
